Allied Benefits Summary
Time Off

Vacation Time

10 Days (Earning .83 days per month worked up to a maximum of 10 per year.  Eligible after 6 months.  After 5 years employment, eligible for 15 days per year.  After 10 years employment, eligible for 20 days per year.)

Sick Time

5 Days (Earning ½ day per month to a maximum of 5 per year; eligible after 90 days.)

Personal Time

2 Days (Earning 1 day every 6 months to a maximum of 2 per year.  Employees hired prior to July 1st may take two personal days during that calendar year.  Employees hired between July 1st and September 1st may take one personal day after their 90 day introductory period but prior to Dec. 31st of that calendar year.)

Empire Blue Cross Blue Shield

Full-time employees eligible after 30 days of employment.

EPO:

  • $39.75 biweekly single
  • $103.05 biweekly family
  • Prescriptions: $10 co-pay generic/$25 brand name formulary/$50 non-formulary
  • No option to go out of network
  • No referrals necessary from Primary Care Physician to see Specialists, etc.

Full-time employees eligible after 30 days of employment.

PPO:

  • $56.73 biweekly single
  • $147.20 biweekly family
  • Prescriptions: $10 co-pay generic/$25 brand name formulary/$50 non-formulary
  • Option to stay in network with $20 co-pay or go out of network and incur deductible and coinsurance

Full-time employees eligible after 30 days of employment.  This is a voluntary, non-contributory plan.

Vision Care

$5 co-pay for one examination every 24 months, $10 co-pay for select frames, $25 co-pay for contact lenses, $35 allowance for non-plan eyewear purchase.

Life Insurance in the amount of 1.5 of the employee’s annual salary (minimum $50,000; maximum $500,000) effective on the employee’s 31st day of service.  Additional Life Insurance available at employee’s expense.

Short Term Disability

Benefits calculated as a percentage of salary up to a maximum of $170 per week with a maximum of 26 weeks.  (NJ maximum of $459 per week, CA maximum of $728 per week).

Supp. Short-Term Disability
Aflac

Available at employee’s expense.  Monthly benefit from $500 - $5,000, subject to income requirements and benefit restriction.  Benefit period of 3 months.  Elimination Period (Accident/Sickness): 0/7, 0/14, 7/14.

Long-Term Disability

Monthly benefit equals 60% of basic monthly earnings to a maximum of $15,000.
